Admissions Application

Filling out an online application is another step forward in the Admissions process. We hope before applying your student has visited the campus and arranged to shadow for a day. If you’d like to schedule either, please contact our Admissions Team.

All applications are reviewed on a rolling basis. High School applicants seeking first round admission must apply by December 1, 2024.

After completing an application, our Admissions Team will request the following records to be sent to the Elms:

  • Current year’s report card

  • Last year’s final report card

  • Standardized Test Scores

  • Attendance & Disciplinary Records

  • Accommodations

Pre-School - Grade 4
Parents/guardians with students applying to pre-school, pre-kindergarten and kindergarten will also fill out a questionnaire so we can learn more about your family. This should take 5-10 minutes.

Middle School & High School (Grades 5+)

In addition to completing the Middle School or High School application, students will be asked to complete 2 writing prompts. Parents/guardians will be asked to complete a Family Community Statement. A Record for Records will be sent to your current school upon submission of your completed application by our Admissions Team.

Current 8th Grade Students at Parish Catholic Schools
The admissions process is slightly different for current 8th grade students at parish Catholic schools. Students will receive the Cleveland Diocese’s application from their 8th grade teacher to apply for admissions in January. On that application you’ll list Our Lady of the Elms as your 1st choice. Please keep in mind that your application and records will be sent only to your 1st choice school. Families will be notified of decisions by February 9, 2023, 3PM.
